It seems simple. But on an ordinary Tuesday, your practice works like this:
23:14
The last patient left at 7 PM. You’re still seeing patients.
But now it’s through WhatsApp. “How much is the appointment?” “Do you accept my plan?” “Do you have availability Saturday?” “Can I switch Thursday for Friday?” You answer one by one from the couch, because anyone left without an answer books with another doctor.
On Thursday, two patients didn’t show up. They gave no notice. You paid for the room anyway, and the time slot three other people wanted stayed empty.
You even thought about asking for payment upfront. But you hesitated: it feels like you’re selling an appointment. Is that even allowed?
of booked appointments end in no-shows. And 42% of private clinics say they have difficulty just confirming appointments.Review of PUC-Rio studies; Conclínica survey.
It’s not because you’re disorganized.
Everything outside the practice
No one taught you about everything outside the practice.
Medical school and residency prepared you for the patient in front of you. Scheduling, billing, confirmations, and receipts were left out of the curriculum. And when you looked for a system, you found one built for large clinics:
- charged per doctor, with every colleague who joins the practice increasing the bill;
- WhatsApp sold in message bundles;
- payments, invoices, and reminders as separate modules;
- and, when it doesn’t work, a cancellation process no one answers.
An internal survey shows that 31% of complaints about medical scheduling apps concern apps that crash.**Internal survey, 2026.
You don’t need another system. You need one that handles inquiries, books appointments, and, if you want, leaves the appointment guaranteed.
How it works
You decide whether the appointment arrives guaranteed.
Asking for payment upfront is an option, not a rule. You turn it on for whichever appointments you want (only first appointments, only private-pay appointments, only the most sought-after times) and choose the amount: the full appointment or part of it. The patient secures the time slot via Pix, in the same conversation where they booked. You don’t ask anyone for anything.

- 01
The patient messages the practice on WhatsApp.
RaVir IA responds instantly with available times, the price, address, and the health insurance plans you accept for reimbursement.
- 02
They choose the time slot and, if you enabled it, receive the Pix payment request.
Along with it comes the phrase that takes the pressure out of the conversation: “to secure your time slot.” No one needs to talk about money. Without Pix, the appointment goes on the schedule the same way.
- 03
Paid, secured.Booked, added to the schedule.
The appointment appears on your schedule with a green dot, the money reaches your account, and the patient receives confirmation. The appointment appears confirmed on your schedule and the patient receives a reminder the day before.
- 04
Can’t make it? Reschedule.
The patient reschedules in the same conversation or gets a refundThe time slot becomes available again immediately, and anyone on the waitlist is notified.
Anyone who secured the time slot has one more reason to come, or to give notice in time. The silent no-show becomes a rescheduling, and the time slot that would have stayed empty returns to the schedule.
And before you ask: yes, this is allowed.
Within CFM rules
Charging upfront is allowed. Keeping the money when someone is a no-show is not.
The Brazilian Code of Medical Ethics prohibits charging or keeping money for an appointment that did not take place (art. 59). And CREMEC has already answered the question you may have been afraid to ask: receiving payment upfront is allowed, as long as the doctor refunds or reschedules (opinion CREMEC 20/2020).
Dr. BookPay was designed around this principle. There is no cancellation penalty for a no-show, no withholding, and no button to "keep the money." There is rescheduling and refunding, with one tap.
What Dr. BookPay never does
- Charge a cancellation penalty to a no-show.
- Keep payment from someone who couldn’t make it.
- Answering health questions on WhatsApp. RaVir IA forwards any clinical question to you.
You are not selling an appointment. The patient is securing their time slot.
And with or without Pix, the rest of your practice keeps moving:
What comes with it
Everything a receptionist would do, without hiring one.
All of this works even if you never charge anything upfront.
RaVir IA handles your WhatsApp.
Price, availability, address, and insurance reimbursement plans. Books, confirms, and reschedules, always from the practice’s number. Even overnight.
Reminder the day before and on the day.
The patient confirms; anyone who can’t make it reschedules in advance.
A waitlist that works by itself.
When a time slot opens, everyone waiting finds out.

The patient says how they will be seen when booking.
Everything on your phone.
The app does everything the computer does: schedule, patients, payments, and withdrawals.

Most systems charge per doctor and sell WhatsApp as a package. Here, adding a second doctor to the practice doesn't change the bill, and RaVir IA is already included.
And if it doesn’t work for you? You can leave the same day.

Questions
Questions from people considering switching.
Am I required to charge upfront?
No. Charging upfront is an option: turn it on for whichever appointments you want, or none at all. Without it, Dr. BookPay schedules, confirms, reminds, and reschedules in the same way, with no fee.
Is it ethical to ask for payment before the appointment?
Yes, as long as you refund or reschedule if the appointment does not take place. That is what CREMEC answered in opinion 20/2020, and it is the only way Dr. BookPay charges.
What if the patient doesn’t show up?
Dr. BookPay does not keep anyone's payment, because the Brazilian Code of Medical Ethics does not allow it. The patient reschedules or gets a refund. The difference is that someone who secured the appointment usually gives notice in advance, and the time slot returns to the schedule in time for someone else to use.
Will RaVir IA answer health questions?
Never. It schedules, confirms, reschedules, sends receipts, and, if you enabled it, charges. Any question about symptoms, medication, or tests is passed to you, and the patient is told that you will answer.
Will the patient know it’s an AI?
To the patient, RaVir is the practice's receptionist, with whatever name you choose. If someone asks, it says it is the virtual receptionist.
I accept health insurance. Is it right for me?
It is. Patients with health insurance book the same way, without Pix. When booking, they specify their plan, and the schedule separates private-pay and health insurance appointments.
I already have a receptionist. Does this still make sense?
Yes. They stop answering price and availability questions on WhatsApp and take care of people at reception. They use the same app, with the access you give them.
Do I need to change my WhatsApp number?
No. Messages come from the practice’s number.
How long does it take to get started?
About 5 minutes: availability, addresses, and appointment price. If you want, we can set it up together with you.
When does the money arrive?
Pix arrives instantly. Card payments arrive on the due date of each installment.
What if I want to leave?
One click. No cancellation penalty, no call trying to convince you to stay.
Your next appointment can arrive guaranteed.
